Four Vehicle Fatal Crash in St. Tammany Parish

On Saturday, May 17, 2008 at 7:10 PM, Louisiana State Police, Troop L investigated a four vehicle fatal crash on LA 22 westbound at milepost 61.6, west of Madisonville. The crash involved a 1980 Harley Davidson motorcycle driven by Donald R. Chaffin (W/M, 53 yrs) of Madisonville, LA, a 2008 Harley Davidson motorcycle driven by David R. Green (W/M, 46 yrs) of Brandon, Miss, a 2000 Dodge Neon driven by Sarah D. Haydel (W/F, 24 yrs) of Mandeville, LA and a 1996 Chevrolet Trailblazer driven by Shirley F. Hanks (W/F, 62 yrs) of Metairie, LA. The Dodge Neon was stopped / yielding westbound on LA 22, preparing to turn left into a private driveway. Both Harley Davidson motorcycles, which were traveling westbound on LA 22 behind the Dodge, failed to stop striking it in the rear. The impact caused the Dodge to enter the eastbound lane of LA 22, entering the path of the Chevrolet before swerving back into the westbound lane. After impact, Donald R. Chaffin and David Green were ejected from their motorcycles. Mr. Green was ejected onto the eastbound lane of LA 22, directly in the path of the Chevrolet. The driver of the Chevrolet was unable to stop and ran over Mr. Green. Mr. Chaffin was ejected onto the westbound lane of LA 22.

Mr. Green suffered fatal injuries in the crash and was pronounced dead on the scene. Mr. Chaffin was transported to St. Tammany Hospital in critical condition. Neither the driver of the Dodge or Chevrolet was injured in the crash.

The drivers of the Chevrolet and the Dodge were wearing their seatbelts at the time of the crash and driver impairment not considered a factor on their part.

The drivers of the motorcycles were wearing their helmets at the time of the crash, but driver impairment is unknown at this time. (Results pending toxicology examinations.)

Trooper Rayford is continuing his investigation of the crash.
