| LOUISIANA STATE POLICE COURTESY LOYALTY SERVICE News Release Breaux Bridge Woman Arrested for Insurance Fraud April 1, 2008 On March 4, 2008, Louisiana State Police Insurance Fraud unit received a criminal complaint from Aflac Insurance Company. Detectives began to investigate the allegations made by the insurance company. The investigation revealed that during the time period from March 29, 2006 until October 13, 2006, Mary L. Leblanc of Breaux Bridge, La, had fabricated numerous physician invoices for radiation and oral chemotherapy treatments which she never received. Leblanc submitted these invoices for payment and, as a result defrauded Aflac of $43,304.63. Based on the facts revealed in the investigation, detectives obtained an arrest warrant for Mary Leblanc. Leblanc voluntarily surrendered herself to detectives at the St. Martin Parish Jail where she was properly booked.
