The initial investigation revealed a 1999 Lincoln Town Car, driven by Lyons, was traveling northbound on LA Hwy 17. While attempting to pass another vehicle, Lyons lost control of her vehicle and traveled off the road into a ditch. After driving through the ditch, the vehicle struck a utility pole.

Lyons, who was properly restrained, was airlifted to a local hospital with serious injuries. A short time later at the hospital, she succumbed to her injuries and was pronounced dead.

A toxicology sample was obtained and submitted for analysis. The crash remains under investigation.

In 2021, Troop F has investigated 17 fatal crashes resulting in 20 deaths.
